However I gathered courage and explained to him that I was unable to clearly make out questions due to his accent. He laughed and made extra effort to speak slowly and clearly, he made sure I felt comfortable and that I understood the question. Everything went well after that although the interview lasted longer than expected ;).
To summarize, I would suggest you first put effort from your end to try and understand. If you still have a problem understanding, politely explain it to your interviewer. I am sure that anyone would react the same way as my interviewer did.
What do you do, when speaking to a colleague whose accent is too thick? What do you do when speaking to an external client whose accent is too thick? You ask them to repeat slowly until you understand. You dont give up on them do you..? If you do give up on them, then that is a red flag anyway.
Interviewers will take this into account. If they can see that more time was spent on explaining the questions because of communication gaps, that will be taken into account in the interviewer's feedback.
